Fastest Manmade Object

Question

I remember reading somewhere that during the cold war, a nuclear weapon was placed underground with a manhole cover on-top and then detonated. I'm pretty sure the manhole cover then went orbital however I can't remember any more details. Was this the fastest object launched from earth? What allowed it to reach such speeds? If not, what was?
