How to investigate the cause of a 100% CPU event that lasted for hours?

Is the system load tied directly to CPU usage?
It's a dual-core machine and the load is around 24 but CPU usage is bouncing around from 60-80% and never reaching 100% so how can there be that many processes waiting? I/O is pretty much nill since everything is being read from memory databases…

High CPU interupts when CPU usage is very low

Is it normal for CPU Usage and System Idle Process not to match?
This is on (an old) single-core a Win2003 box. I noticed that, under Task Manager>Performance, CPU usage was topping out at 80% but under Processes the Idle process is showing around 60-70%, with dips and spikes, of course. Quite odd.
